Resumen

Antecedentes. El adenocarcinoma colorrectal (ACCR) es un problema mundial de salud pública, debido a su alta prevalencia y mortalidad. El tratamiento del ACCR es la cirugía, y en etapas avanzadas, ésta se combina con quimioterapia. Uno de los esquemas más frecuentemente utilizados es FOLFOX-6; que incluye 5-fluorouracilo, oxaliplatino y leucovorina (5FUOL). A pesar de que FOLFOX-6 es uno de los mejores esquemas, disponibles actualmente, más del 90% de los pacientes tratados con éste y otros esquemas desarrollan fármacorresistencia, y debido a ello, la mayoría de ellos mueren en un término promedio de 20 meses. Se ha señalado a las células madre de tumor (CMT) como las principales responsables de la iniciación de cáncer, de la invasión de tejidos y metástasis, de su fármacorresistencia, y de su recurrencia. Las CMT tienen marcadores típicos para su identificación; p.e. CD44 y CD24. Sin embargo, estos marcadores no son específicos de CMT. Respecto al surgimiento de fármacorresistencia, nuestro grupo de trabajo, previamente identificó la sobreexpresión de los genes KTR18, ABCB1 y BNIP3 en aislados de CMT resistentes a 5FUOL. El problema. Los tratamientos convencionales son iatrogénicos y los pacientes mueren en un plazo relativamente corto porque surgen CMT resistentes a los fármacos empleados. Por ello se requiere desarrollar métodos, que permitan predecir si un tratamiento estandarizado será útil para al menos prolongar la vida de los pacientes. En caso contrario, permitir la prescripción de un tratamiento más eficaz. Sin embargo, es realmente difícil, laborioso, tardado y costoso aislar CMT y caracterizar su exoma. Justificación. Este estudio permitirá analizar cortes histológicos mediante inmunohistoquímica, simplificando considerablemente la detección de CMT resistentes a 5FUOL. Objetivo. Investigar si, en cortes histológicos de ACCR existe correlación entre la cantidad de CMT ABCB1+, CK-18+, BNIP3+, con las siguientes variables: resistencia in vitro a 5FUOL, presencia de metástasis, índice de Karnofsky y porcentaje de pacientes sobrevivientes a dos años de haberse iniciado el tratamiento. Material, métodos y resultados. De seis ACCRs frescos, mediante cultivo en un medio selectivo para CMT y en placas de baja adherencia, aislamos, expandimos y caracterizamos células resistentes a 5FUOL. Con inmunofluorescencia de doble marcaje y utilizando anticuerpos monoclonales, verificamos que los aislados de ACCR coexpresaban los marcadores de CMT (CD24 y CD44) y de resistencia a 5FUOL (ABCB1, CK-18+ y BNIP3). Además, indujimos a los aislados de CMT a diferenciarse a células epiteliales y verificamos su estirpe epitelial, porque estas células expresaban los marcadores de epitelio CK-17, CK-20, una mezcla de citoqueratinas (Cocktail-CK) y EGFR. Por lo tanto, demostramos que las CMT de ACCR expresaban los marcadores de resistencia y que éstas podían identificarse en cortes de ACCR, utilizando estos marcadores. De la base de datos de nuestro laboratorio seleccionamos 20 casos de ACCR, registramos los datos sociodemográficos y clínicos y recuperamos las muestras de tumor correspondientes — criopreservadas o incluidas en parafina —, de las cuales obtuvimos cortes finos para determinar el área ocupada por las CMT que coexpresaban los marcadores de CMT resistentes a 5FUOL, previamente validados en los aislados de CMT, arriba mencionados. Cuantificamos en cada corte, el área ocupada por las células coexpresando los marcadores de interés. Para ello utilizamos un programa de cómputo que dividió en cuadrados el área completa, ocupada por cada corte y dos observadores contaron, independientemente, los cuadros positivos para cada marcador o mezcla de marcadores. El número de cuadros se correlacionó o asoció con las variables arriba mencionadas, utilizando pruebas estadísticas apropiadas. La cantidad de CMT coexpresando CD24/CD26, ABCB1/CK18 y ABCB1/BNIP3 o sólo ABCB1 en cortes de ACCR resistentes a 5FUOL, mostró una correlación y dependencia positivas (p<0.05) con todas las variables arriba mencionadas. Conclusión. Nuestros resultados apoyan nuestra hipótesis. Por lo tanto, la cuantificación de CMT ABCB1+/BNIP3+/KRT18+, en cortes histológicos de ACCR permite saber si FOLFOX-6 será eficaz para tratar a los pacientes con ACCRs desde antes de instituir este tratamiento. El enfoque aquí presentado puede extenderse a otros tipos de cáncer con resultados promisorios. ABSTRACT Background. Colorectal adenocarcinoma (CRAC) is a worldwide public health problem due to its high prevalence and mortality. The treatment of CRAC is surgery, and in advanced stages, surgery is combined with chemotherapy. One of the most frequently used schemes to treat CRAC is FOLFOX-6; which includes 5-fluorouracil, oxaliplatin and leucovorin (5FUOL). Although FOLFOX-6 is one of the best schemes currently available, more than 90% of patients, treated with this and other schemes, develop drug resistance. As a result, most patients die within an average of 20 months. Tumor stem cells (CMT) have been identified as the main responsible for the initiation of cancer, tissue invasion and metastasis, drug resistance, and recurrence. The CMT have typical markers for identification; e.g. CD44 and CD24. However, these markers are not specific of CMT. Regarding the emergence of drug resistance, our working group previously identified the overexpression of the KTR18, ABCB1 and BNIP3 genes in CMT isolates resistant to 5FUOL. The problem. Conventional treatments are iatrogenic and patients die in a relatively short time due to the emergency of CMT drug-resistant to the treatment given to the patient. Therefore, it is necessary to develop methods that allow predicting whether a standardized treatment will be useful to at least to enlarge the timelife of patients. Otherwise, to allow prescribing a more efficatious treatment. However, it is really difficult, laborious, time consuming and expensive to isolate CMT and characterize their exome in each patient. Justification. This study will allow the analysis of histological cuts by means of immunocytochemistry, considerably simplifying the detection of CMT resistant to 5FUOL. Objective. To investigate if, in histological cuts of ACCR, there is a correlation between the amount of CMT ABCB1+, CK-18+, BNIP3+, with the following variables: in vitro resistance to 5FUOL, presence of metastasis, Karnofsky index and percentage of survival patients, after two years of initiated their treatment. Material, methods and results. From six fresh ACCRs, we isolated, expanded and characterized cells resistant to 5FUOL by culturing them in a selective medium for CMT and using plates of low adherence. With doublelabeled immunofluorescence and using monoclonal antibodies, we verified that the ACCR isolates coexpressed the markers of CMT (CD24 and CD44) and resistance to 5FUOL (ABCB1, CK-18 + and BNIP3). In addition, we induced the CMT isolates to differentiate into epithelial cells and verified their epithelial linage, because these cells expressed the epithelial markers CK-17, CK-20, a mixture of cytokeratins (Cocktail-CK) and EGFR. Therefore, we demonstrated that the CMTs isolated form CRAC expressed resistance markers and that these could be identified in CRAC sections, using these markers. From the database of our laboratory, we selected 20 cases of CRAC, recorded the sociodemographic and clinical data and recovered the corresponding tumor samples — cryopreserved or included in paraffin —, from which we obtained fine cuts to determine the area occupied by the CMT that co-expressed the 5FUOL-resistance markers of CMT, previously validated in the CMT isolates. In each histological cut we quantify the area occupied by the cells coexpressing the markers of interest. To do this, we used a computer program that divided into squares the entire area occupied by each cut. Then, two observers independently counted the positive squares (showing a specific color for each marker of pair-markers). These correlation or dependence of the area occupied by each marker or pair-markers was analyzed with the above-mentioned variables, using appropriate statistical tests. The area (equivalent to the number of CMT) co-expressing CD24/CD26, ABCB1/CK18 and ABCB1/BNIP3 or only ABCB1 in ACCR sections resistant to 5FUOL, showed a positive correlation and dependence (p <0.05) with all the variables mentioned above. Conclusion. Our results support our hypothesis. Therefore, the quantification of CMT ABCB1+/BNIP3+/KRT18+, in histological cuts of CRAC allows knowing if FOLFOX6 will be effective to treat patients with CRAC before instituting this treatment. The approach presented here can be extended to other types of cancer with promising results.
