Resumen

La infección con el virus de la hepatitis C (VHC) es un problema de salud pública a nivel mundial y es considerada como una de las principales causas de hepatitis crónica, cirrosis y carcinoma hepatocelular. En México; aproximadamente el 1.2 % de la población se encuentra infectada con el VHC. Actualmente, la única terapia disponible es el tratamiento combinado de Interferón alfa pegilado y ribavirina; sin embargo, solo un 50-55% de los pacientes logra una respuesta sostenida al final del tratamiento. La terapia génica constituye una alternativa prometedora en el tratamiento contra VHC; y existen diversos estudios en los que se ha logrado un bloqueo exitoso de la expresión y replicación del virus. El empleo del RNA de interferencia (RNAi) como terapia contra el VHC ha sido de gran interés debido a su gran potencial terapéutico y alta especificidad. En este trabajo se estableció como principal objetivo inhibir la expresión y replicación del RNA del Virus de la Hepatitis C en un modelo “in vitro”, a través del bloqueo selectivo de una secuencia conservada del gen NS5b, mediante la tecnología de RNA de interferencia. Se utilizó la línea celular de hepatocitos Huh-7-VHC (línea celular que expresa establemente un replicón subgenómico del VHC), la cual fue transfectada con 3 siRNAs seleccionados contra diversas regiones del gen viral NS5b de manera individual y combinada y se analizó la inhibición de la expresión de la polimerasa viral y la replicación del genoma del VHC. Las células tratadas con el siRNA “A” mostraron una mayor inhibición en los niveles de RNA viral a los 6 días post-transfección, con inhibiciones de 86 y 88% con 100 y 200 nM. En cuanto al siRNA “B”, no se observan cambios significativos en los niveles de RNA viral a los 2 días pos-transfección, mientras que a los 4 y 6 días postransfección se obtuvo un aumento significativo en el RNA viral con respecto al control. Con el siRNA “C” se observó inhibición significativa a los 2 y 4 días post-transfección de alrededor del 57%, sin embargo dicha reducción no se observa a los 6 días post-transfección. Al realizar las combinaciones de estos siRNAs no se observó una ventaja al utilizarlos de manera combinada, obteniéndose una menor inhibición viral al utilizar el siRNA “A” de manera individual. En base a los resultados obtenidos concluimos que el siRNA “A” fue el que presentó la mejor inhibición del VHC de los 3 siRNAs evaluados, y al compararlo en forma individual y combinada nos aporta datos útiles en la compresión del mecanismo del RNA de interferencia. Abstract The Infection with Hepatitis C virus (HCV) is a worldwide public health problem and it is considered as one of the main causes of chronic hepatitis, cirrhosis and hepatocellular carcinoma. Approximately 1.2% of the population in Mexico is infected with HCV. Currently the only therapy available is the combined treatment of pegylated interferon and ribavirin; nevertheless only 50% to 55% of the patients present a sustained positive response by the end of the treatment. Genetic treatments constitute a promising alternative for the treatment against HCV and there have been several studies in which a successive blocking of the expression and replication of the virus has been achieved. The use of the RNA interference (RNAi) as a therapy against HCV has been of huge interest due to its great therapeutic potential and high specificity. The main objective of this work is to inhibit the expression and replication of the RNA of the Hepatitis C Virus in an “in vitro” model by selectively blocking a preserved sequence of the NS5b gene using the RNA interference technology. A Huh-7-VHC replicon cell (cellular line that expresses a stable subgenomic replicon of HCV) was used; it was transfected, individually and combined with 3 selected siRNAs against diverse regions of the viral gene NS5b; then inhibition of the polymerase viral expression and the replication of the HCV genome was analyzed. The cells that were treated with the siRNA “A” presented a higher inhibition of the viral RNA levels during the 6 days post-transfection with inhibitions of 86% and 88% with 100 and 200 nM. The siRNA “B” no presented changes in the viral RNA 2 days after the transfection and at the 4th and 6th day after the transfection an increase in the viral RNA was obtained compared to the control. With the siRNA “C” an inhibition was observed at the 2nd and 4th day post-transfection of 57%, never the less this reduction can’t be observed at the 6th day post-transfection. No advantage was observed when combining the siRNAs obtaining a minor viral inhibition when using the “A” siRNA individually. Based on the results obtained, we conclude that the “A” siRNA presented the best HCV inhibition of the 3 siRNAs evaluated, and when compared individually and combined it gives useful data in the RNA interference mechanism comprehension.
